JOB PURPOSE:
The pharmacy care coordinator assists the population health ambulatory care pharmacist and multi-disciplinary population health team in providing service to patients and physicians in accordance with all applicable laws and regulations as well as company philosophy, policies and service standards. The pharmacy care coordinator is supervised by the pharmacist on duty and supports performance in our value-based contracts.

KEY RESPONSIBILITIES:
1. Supports and assists with value-based care pharmacy-related measures for specified value-based contracts to include but not limited to medication adherence, 90-day supply, brand formulary compliance and cost savings opportunities.
2. Maintains confidentially of all information related to patients and medical staff.
3. Actively engages patients and clients with respect to customer service/program integrations offered by the multi-disciplinary population health team.
4. Reaches out to patients within timeline required by departmental and regulatory guidelines. Organizes family meetings and/ team conferences where necessary to ensure patients’ needs are met.
5. Make outgoing calls and receive and respond to incoming patient calls; take appropriate action based off protocols and use critical thinking skills to escalate calls to appropriate parties when needed
6. Assists population health team with various value-based contracts initiatives.
7. Answers questions and provides information to population health team as able regarding medication access and assistance.
8. Schedules patient appointments for pharmacist and care management and assists with the chart documentation of services.
9. Performs other duties as assigned
KNOWLEDGE, SKILLS, ABILITIES
• Proficient typing skills and medical terminology
• Proficient pharmacy/drug terminology, brand/generic, medication classes
• Teamwork attitude with ability to work with many types of medical professional
• Excellent telephone manners and customer service skills
• Well organized with the ability to balance multiple tasks and prioritize workload
• Understand and articulate the pharmacist’s clinical role in the care of patients


Requirements

MINIMUM EDUCATION REQUIRED:
High school diploma or GED required.
Pharmacy Technician or Medical Assistant certification required.
MINIMUM EXPERIENCE REQUIRED:
Two (2) years retail pharmacy experience OR healthcare experience with direct patient contact required.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Registration (current) required with the state Board of Pharmacy as a Pharmacy Technician or Pharmacy Intern OR certification from the American Association of Medical Assistants (AAMA) or a similarly recognized certifying organization required.
ADDITIONAL QUALIFICATIONS:
Previous experience with hospital pharmacy preferred.
Experience with patient assistance programs preferred.
Bilingual English/Spanish preferred, but not required.
