The PEM Care Management department is made up of a diverse group of individuals. Each come with their own unique backgrounds, skills, and experiences. The department is comprised of Social workers, RNs, and CMAs. Together we have built a team that can draw on each other’s strengths and supplement each other’s weaknesses. We expand each other’s knowledge and skills through collaboration and coaching, and provide opportunities for growth within the department and organization. We work together as a team every day to expedite the most appropriate and timely discharges for our patients.  We work collaboratively with other members of the interdisciplinary team, as well as post-acute providers to ensure continuity of care. We do this and so much more everyday as a team!

 

JOB PURPOSE: As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families /caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.


Requirements

MINIMUM EDUCATION REQUIRED: High School diploma, or the equivalent such as the General Education Development (GED)

MINIMUM EXPERIENCE REQUIRED: Two (2) years of experience in a healthcare or social services setting

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: None

ADDITIONAL QUALIFICATIONS: Associate's degree or two years of college in human services/health related discipline preferred. Knowledge of community-based / post-acute care resources such as primary care, skilled nursing facilities, home health, durable medical equipment, and transportation.
