Piedmont Atlanta - Emergency Department - Post Discharge Call LPN

Schedule: Full-time (8 hr Shifts), Mon-Fri

This is a full-time in-person role; remote work is not considered

 

JOB PURPOSE:
Places telephone calls to every discharged patient unless patient designated they preferred no phone call for patients dismissed from Emergency and Medical Surgical Services. Follows the scripted process outlined for discharge phone calls. Promotes service-oriented processes with all interactions. Indirect total, holistic nursing care of adolescent, adult and geriatric patients.


Requirements

MINIMUM EDUCATION REQUIRED:
Graduate of a LPN program


MINIMUM EXPERIENCE REQUIRED:
Three (3) years of current clinical experience in acute health care setting with Medical, Surgical and Emergency Department experience preferred.


MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a LPN or NLC/eNLC Multistate License.


ADDITIONAL QUALIFICATIONS:
Understanding of disease processes and current medication knowledge as well as attention to detail. Must be service oriented, critical thinker, work well with multiple departments and people and possess excellent communication skills with a positive vocal delivery. Current certification in BCLS.


KEY RESPONSIBILITIES:
1. Calls all discharged patients on a daily basis.
2. Maintains orderly process and organization for post-discharge phone calls.
3. Contributes to process improvement in nursing/physician care delivery.
4. Maintains statistical data related to discharge program.
5. Attends meetings as required by department.
6. Assumes responsibility to maintain own current and complete competency file.
