Description

JOB PURPOSE: Reviews, analyzes, and codes documentation for hospital inpatient medical records to select and sequence the appropriate ICD-9-CM diagnosis, ICD-9-CM procedure codes, ICD-10-CM and ICD-10-PCS as applicable to the transition to ICD-10. Primary coding responsibility is Inpatient Coding.

KEY RESPONSIBILITIES: 1. Assign diagnostic and procedural codes to patient records using ICD-9-CM, ICD-10-CM and any other designated coding classification system in accordance with the UHDDS coding guidelines. 2. Assign procedural categories as applicable. 3. Assign appropriate discharge disposition as necessary. 4. Maintain or exceed established coding accuracy and productivity standards. 5. Initiates physician queries in compliance with PHC’s policies where appropriate. 6. Ensure demographic information and documentation is congruent with scheduled or performed procedures. 7. Evaluate assigned Coding work queue and work with the appropriate revenue cycle/hospital team to resolve accounts with the expected resolution timeframe.

KNOWLEDGE, SKILLS, ABILITIES Extensive knowledge of ICD-9-CM/ICD-10-CM and ICD-10-PC, MS-DRG’s and POA Assignments coding principles and guidelines. Knowledge of medical terminology, abbreviations, techniques and surgical procedures; anatomy and physiology; major disease processes; if applicable pharmacology; and the metric system. Knowledge of reimbursement systems, as well as federal, state and payer-specific regulations and policies pertaining to medical documentations, billing and coding. Knowledge of Standards of Ethical Coding. Skill and ability to communicate effectively both orally and in writing. Skill and ability to research and analyze data, draw conclusions, and resolve issues.

Requirements

MINIMUM EDUCATION REQUIRED: High school diploma or equivalent required.

MINIMUM EXPERIENCE REQUIRED: One (1) year of coding experience required.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: None. ADDITIONAL QUALIFICATIONS: One or more certifications required - RHIA, RHIT, CCS, CCA, CCS-P, CPC, CPC-A, CPC-H.

Coding Certificate program (AHIMA accredited) preferred. Experience in coding at a multi-facility organization and remote coding experience is a plus.
