JOB PURPOSE:
Provides professional medical social work services ordered by a physician to patients from older adolescent through geriatric age that are necessary to resolve social or emotional problems expected to be an impediment to the effective treatment of the patient’s medical condition or to his or her rate of recovery; may also provide medical social work services to the patient’s family member or caregiver on a short-term basis when that service is necessary to resolve a clear and direct impediment to the effective treatment of the patient. Conducts medical social work evaluations and works with the patient, family, representatives (if any) and caregivers to develop a plan to secure safe and effective living arrangements that meet the medical and social needs of the patient, support activities of daily living and avoid life crises. Practices in compliance with professional knowledge and standards of clinical practice and in accordance with policies and procedures of the agency, laws and regulations of Medicare, Medicaid and other third party payers. Demonstrates professional growth and accountability. Reports to and is supported by the Piedmont Home Health Clinical Manager.

MINIMUM EDUCATION REQUIRED:
Master’s degree from an accredited social work program required.
MINIMUM EXPERIENCE REQUIRED:
One year social work experience in a health care setting required.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current GA medical social work license. Current CPR certification. Current valid GA driver’s license.
ADDITIONAL QUALIFICATIONS:
Experience in an acute care setting preferred. Knowledge of community resources preferred.
